I was the first person to walk in after this incident, well, me and the Wing Commander. As a matter of fact, the pictures you see on the internet that were taken at NIGHT were taken by me and another QA Inspector. I was working for QA at the time and was involved with the AIB. As was previously stated, the crew chief didn't have the holdback properly attached. As a matter of fact he was told, as he sat in the cockpit, that it wasn't even hooked up prior to the run, and he told the airman on the ground to hook it up. The airman had little experience with this task and asked the engine run crew chief to look at it, but the crew chief said he was sure it was fine. F-15 Engine Troop was accurate with everything with the exception of the number of afterburner stages. While the F100-PW-220 DOES have 5 stages, the F-15E model has F100-PW-229 which has 7 stages.
